 **All Guests must be current on their vaccines and Spayed or Neutered and on flea and tick preventative.**

Here you will find all the forms you need to make a reservation at the hotel.

NEW GUESTS WILL NEED TO FILL OUT FORMS 1, 2, 3, 4 and if he/she is on medication,  #5.
RETURNING GUESTS NEED ONLY TO FILL OUT #6 if all info remains the same on prior forms.

 1. MASTER BOARDING AGREEMENT: This is a form required by any quality care facility. 

2. AUTHORIZATION TO RELEASE PET RECORDS   This form will be faxed to your veterinarian.  This protects your baby against unvaccinated or unhealthy contagious guests.  All our guests are required to be currently vaccinated and healthy. See our vaccination schedule below. For the health and security of all guests, this form MUST be faxed from your veterinarians office.

3. PET ASSESSMENT FORM  This form will help us understand your baby's personality and allow us to insure your baby's stay is a pleasant one.

4. RESERVATION REQUEST FOR NEW GUESTS This form gives us the info we need regarding the baby's parents contact information and check in and out dates and times.

5. MEDICATION FORM If your baby is on regular medication this form is REQUIRED. We are happy to dispense oral or topical medication at no charge per your written instructions on this form.

6. RETURNING GUESTS RESERVATION REQUEST FORM If none of your baby's info has changed within the past year,(Vet, health status, feeding needs, medication, behavioral, etc)  this is the only form returning guests are required to fill out.

All forms will be required to be renewed yearly or as needed.

REQUIRED VACCINATIONS AND SCHEDULE

For the health and safety of our guests, all dogs must have up-to-date records for all of the vaccinations Hotel Poochie requires. We do not accept home vaccinations or titer tests in lieu of vaccinations for any guests. Vaccinations must be administered at least 14 days prior to arrival under the guidance of a licensed veterinarian. Written proof mailed or faxed from the veterinarian of the following are required for all guests.

Vaccinations:

DPP*             One or three year vaccine required
Bordetella    Required every 12 months, suggested every 6 months for your dogs optimum health. Please note, many veterinarians administer this vaccine only by request.
Rabies           One or three year** vaccine required

All Guests should be flea and tick free. Guests with signs of fleas and/or ticks will be treated at the grooming salon at the Guest's Parent's expense prior to check-in.
